constipation. The patient also complained that he had both abdominal pain and a feeling of
fullness in his rectum. He mentioned that when he goes to the bathroom, he is not able to fully
empty his bowel. In addition, he reported the presence of bright red streaks in his stool.
Diagnostic studies including a digital rectal exam as well as fecal occult and fecal
immunochemical tests to detect blood or blood proteins in the stool were ordered. The results of
the tests were positive for the presence of blood in the patient’s stool. Furthermore, the patient
reported to the hospital’s outpatient department next day and a colonoscopy was performed.
During the colonoscopy procedure, a tumor was found in the lower rectum. A biopsy procedure
was performed and a sample of tissue was obtained from the tumor and was sent to the pathology
department. The result of this diagnostic test came back the next day and was positive for low
rectal cancer. The patient was diagnosed with low rectal cancer. Today, this patient presents for
abdominoperineal resection (APR) of the rectum with reconstruction to follow. Laparotomy
with extensive adhesiolysis to free sigmoid colon taking approximately 45 minutes was
performed. Then, cylindrical APR was performed, along with excision of the sigmoid colon and
resection of the anus. The patient also had colostomy creation at the same time. Remaining
sigmoid colon was bypassed to skin.
