PCP Final Presentation-this is a two- step assignment. Step 1 is submitted here and the presentation is graded for content. Step 2 is submitted to the discussion board for a class participation grade. All steps must be completed and submitted to their proper place by the deadline for each in order to receive credit.
1. For Final Presentation of the Personal Change Plan, you will be reporting on your end-of-semester results. This report will be presented with audio. Use Kaltura which is built into Blackboard, or Screencast-o-matic (see the link below. It is a free download and creates audio presentations from Powerpoint slides). Audio is required to explain each slide. Audio inserted on the slide will not transfer well on Powerpoint presentation when submitting to Blackboard and is not accepted. Therefore, in order to receive credit for audio, students need to use either Kaltura or Screencast-o-matic. You may use other presentation methods as long as they can be viewed by the instructor in Blackboard with prior approval.Powerpoint with audio inserted is not accepted.
It is important to follow the instructions and include the required information. Step 1-the presentation part of the assignment is due on Sunday by 11:59 pm, and is worth 30 points. Step 1 of the PCP Final Presentationshould be submitted through Blackboard for grading. To receive a grade for the presentation it must be submitted using the assignment link and submitted on time! To receive credit for the discussion please submit the presentation to the appropriate discussion board.
The presentation requirements are:
1) Title slide. State the personal change/ healthy behavior you have been working on during this course. Include your name and the SMART goal. (1 slide) 2 points
2) Show the data you have been collecting since the midpoint report. (journal, tracker or excel spreadsheet–1 or 2 slides) 4 points
3) Demonstrate your results using a graph, pie chart or other visual graphic. The graphic should compare all the data from 7 weeks. There should be 7 weeks of data. (line graph, bar graph, some type of graph—1 slide). 8 points
4) Explain what the results mean to you. Were you successful in your personal change plan efforts? Why or why not? (1 slide) 4 points
5) Explain how all the strategies were used. Highlight which strategies for change were more effective. If successful, how will you maintain the change? If unsuccessful, what can you do now to increase the prospects of success? If any were changed demonstrate and explain which ones and why. (1 slide) 4 points
6) Provide advice for anyone who would like to undertake a personal change project like the one you just completed. (1 slide) 4 points
7) The presentation must be interesting and visually appealing to the audience by using color and appropriate graphics. Final results are explained using Kaltura, Screencast-omatic or similar tool in a maximum of 7 slides. 4 points

8) When you have completed the written assignment move on to step two and post the presentation to the appropriate Discussion. When posting your presentation to the Discussion Board you will need to provide a script. Therefore, you may want to create a Word Doc. of what is discussed in the audio portion of the presentation. The script is posted with the presentation on the Discussion Board. Please label your discussion with your name and the health behavior. Respond to two of your classmates.
To receive credit for step one of this assignment (the presentation) it must be submitted to this assignment link and submitted on time. This assignment is not accepted late even if it is submitted to the discussion board on time. I do not grade the contents of the presentation in the discussion board.
9) When steps 1-8 are complete, view ten of your classmates presentation and complete the final summary which is due in week 9.
