CASE STUDY ASSIGNMENT:
ASSESSING THE HEAD, EYES, EARS,
NOSE, AND THROAT
Nurses conducting assessments of the
ears, nose, and throat must be able to identify the small differences between lifethreatening conditions and benign ones. For instance, if a patient with a sore throat and
a runny nose also has inflamed lymph nodes, the inflammation is probably due to the
pathogen causing the sore throat rather than a case of throat cancer. With this
knowledge and a sufficient patient health history, a nurse would not need to escalate
the assessment to a biopsy or an MRI of the lymph nodes but would probably perform a
simple strep test.
Most ear, nose, and throat conditions that arise in non-critical care settings are minor in
nature. However, subtle symptoms can sometimes escalate into life-threatening
conditions that require prompt assessment and treatment.
In this Case Study Assignment, you consider case studies of abnormal findings from
patients in a clinical setting. You determine what history should be collected from the
patients, what physical exams and diagnostic tests should be conducted, and formulate
a differential diagnosis with several possible conditions.

Below is the case study.
CASE STUDY 1: Focused Nose Exam Richard is a 50-year-old male with nasal congestion,
sneezing, rhinorrhea, and postnasal drainage. Richard has struggled with an itchy nose, eyes,
palate, and ears for 5 days. As you check his ears and throat for redness and inflammation, you
notice him touch his fingers to the bridge of his nose to press and rub there. He says he’s taken
Mucinex OTC the past 2 nights to help him breathe while he sleeps. When you ask if the
Mucinex has helped at all, he sneers slightly and gestures that the improvement is only minimal.
Richard is alert and oriented. He has pale, boggy nasal mucosa with clear thin secretions and
enlarged nasal turbinates, which obstruct airway flow but his lungs are clear. His tonsils are not
enlarged but his throat is mildly erythematous.

CC (chief complaint) a BRIEF statement identifying why the patient is here – in the patient’s
own words – for instance “headache”, NOT “bad headache for 3 days”.
HPI: This is the symptom analysis section of your note. Thorough documentation in this section
is essential for patient care, coding, and billing analysis. Paint a picture of what is wrong with the
patient. Use LOCATES Mnemonic to complete your HPI. You need to start EVERY HPI with
age, race, and gender (e.g., 34-year-old AA male). You must include the seven attributes of each
principal symptom in paragraph form not a list. If the CC was “headache”, the LOCATES for the
HPI might look like the following example:

siblings, and children. Include grandchildren if pertinent.

Questions to ask
Using the OLD CARTS, further questioning is needed to determine the patient’s illnesses.
I would as the following questions;

What time of the day do you cough more often? Coughing can occur at any time
of day or night or even after he has taken medication.

How severe is the cough? Rate the degree of cough on a scale of 1-10 with one
being mild and 10 most severe?

Are you allergic to any type of environment? Examples include cold, dusty or any
other environment.

When it comes to colon polyps, inquiries questions around the patient’s bowel
habits and the presence of blood in their stools.

Another thing that I will ask about is whether or not the patient has recently lost
weight.
Physical aspects
As a result of high blood pressure, it is vital to perform a cardiovascular system
assessment. The examinations include taking blood pressure readings and performing chest
examinations, which will most likely include the use of IPPA, a systemic assessment tool for
evaluating the chest (Proctor & Richards, 2020). Inspection, palpation, percussion, and
auscultation are all performed using IPAA. It is also necessary to assess diabetes by monitoring
vital signs, performing neurologic evaluations, performing a funduscopic examination, and

performing foot and hand assessments. The physical examination is significant as it contributes
to the efficiency of care provided.
The likely cause of dry cough
Because of his hypertension, the patient is prescribed lisinopril to help her manage her
high blood pressure. Lisinopril, which the patient has been using for the past six months, is
thought to cause the dry hacking cough that the patient is experiencing. Studies have discovered
that lisinopril is an Angiotensin-Converting Enzyme inhibitor, which leads to an increase in
nitric oxide production, which irritates the lungs and causes dry coughing (Yilmaz, 2019).
Diagnoses to consider
Generally, the aging population is associated with lower and compromised immunity
hence various diagnoses have to be conducted to provide sufficient treatment. In this case, the
other possible diagnosis includes heart failure and diabetes. Diez-Villanueva & Alfonso (2016)
asserts that heart failure is common among older adults. At 75 years, the patient has a high risk
of suffering from heart failure. Another possible diagnosis is diabetes because of his history of
being diabetic.
Other tests
The state of the lungs can only be determined by imaging the chest with X-rays and
computed tomography (CT) scans. The dry cough may be compromising the lungs, and as a
result, it is necessary to assess their state. An electrocardiogram (ECG) is also required to ensure

that the heart is operating properly and efficiently (Yılmaz, 2019). This procedure must be
performed to determine whether a patient has heart failure or not.
Treatment and education
The patient continues to require lisinopril for the treatment of hypertension. However,
because they also lower blood pressure, angiotensin II receptor blockers should be substituted for
the current pharmaceutical regimen. If the medicine is not changed, such as with the use of
Tessalon Perles, it is necessary to educate the patient on how to reduce dry coughs. Tra

When seeking to identify a patient’s health condition, advanced practice nurses can use
a diverse selection of diagnostic tests and assessment tools; however, different factors
affect the validity and reliability of the results produced by these tests or tools. Nurses
must be aware of these factors in order to select the most appropriate test or tool and to
accurately interpret the results.
Not only do these diagnostic tests affect adults, body measurements can provide a
general picture of whether a child is receiving adequate nutrition or is at risk for health
issues. These data, however, are just one aspect to be considered. Lifestyle, family
history, and culture—among other factors—are also relevant. That said, gathering and
communicating this information can be a delicate process.
For this Assignment, you will consider examples of children with various weight issues.
You will explore how you could effectively gather information and encourage parents
and caregivers to be proactive about their children’s health and weight.
